A 19 year old female patient came to the OPD for consultation about her missed periods and subsequent diagnosis of unwanted pregnancy of approximately one and a half month duratio. She was accompanied by her living in partner.
Her relationship was disapproved by her own parents hence she could not take support of them at the time of difficulty in her life.
After counselling , the couple opted for termination of pregnancy by taking tablets.
For the girl, the experience was unpleasant as away from home and security she was dealing with unfamiliar life changing experience.
The couple was also apprehensive about future repercussions on fertility.
Though an expert Gynaecologist will take all necessary precautions to make the procedure safe for the woman, still all medical procedures have their own inherent complications and long term effects. They can be both physical and emotional.

A 24 year old unmarried woman was came to the clinic complaining of bleeding and pain at private parts.
She had attempted intercourse with her fiance for the first time a couple of days back.
She has been in the steady relationship with him over past 4 years.
On examination by the Gynaecologist, it was found that there was injury at perineum (private parts) and she had active bleeding from that site.
She required suturing of the wound and antibiotics.
Though she took the treatment as advised but she was scared that her family should not come to know about her problems as she was going to her hometown for Diwali vacation.
Gynaecologist also needed to give her 'Emergency Contraceptive Pill' to protect her from unwanted pregnancy.

The emotional impact was also significant on the couple. The girl in question was angry at her beau and did not want to either speak to him or continue with the relationship.The guy was both scared and anxious that he would lose the love of his life.
Apart from medical treatment the couple will also require psychological counselling to maintain healthy future relationship.

The point here is that there is need for awareness programmes for the youngsters in our society about sexual health & relationship management. They need to learn to utilise their time effectively in productive activities. The communication gap between parents and youngsters need to be bridged.

Today I want to discuss the emotional and social  dilemma faced by a couple who were facing the situation of untimely pregnancy, hence unwanted pregnancy.

Urban social scenario is changing.
People are getting married in their late twenties.
After marriage too there is a great financial demand on both husband and wife.
To meet these goals and also to get to know each other, they do not want a child soon after marriage.

Sadly very few men and women seek pre-marital or birth control counselling and land up with unwanted pregnancy.

One such couple was seen in the OPD.  Parents of both husband and wife were in their respective hometown.
Now the couple did not have the heart to tell either set of parents about this pregnancy as they feared that they will be pressurised into continuing which they did not want.
Further, they were concerned about future fertility issues as both were nearing 30 years of age.
Matter was further going to be made complex by the arrival of mother-in -law in next week.
If mother-in -law senses pregnancy in her daughter-in-law, the couples decision will get affected which in turn might affect their life -planning.

With lot of anxiety and after deep counselling about the risk and complications and future outcome of next pregnancy planning, they opted for process of D & E- surgical evacuation of pregnancy in uterus.

By grace of God, no complication arose in the case .

The point here is that it may not be so in all cases.
Complications are part of any surgical procedure.

Future fertility may get affected because of the procedure.
Lady may have difficulty in conceiving later because of so many other factors and advancing age leading to subfertility.

Guilt and shame then become a constant in the couple's relationship.

A 32 year old married woman, she has taken abortion tablets one month prior. Bleeding happened normally and the process of abortion was complete.
The next month, again the periods were overdue. 
On checking , the results showed that she is pregnant again.
